As a family member, I appreciate the care and dedication of the nurses, PT, OT, most of the aides and of course the unit coordinator, Todd Hanson. Todd always greeted visitors with a smile and warm greeting. I felt it was a very personalized experience.

I am writing this review as a 5 star for the clinical staff that directly dealt with my family member, who was there for 1 week of rehab. I’ve held a position for many years in healthcare where I directly deal with surgery/recovery of patients. I seek care for my family by professionals who practice on the highest level of patient care. My father’s stay at this facility was not planned. It was an unforeseen bump in the road of his recovery after a knee replacement. You get what you get in situations such as this, as you never know how things will go. I sat with my father in his room every single day. He was there for 8-9 hours a day, so I was able to see a lot that went on in his section of the facility. We were blessed to have been in the care of James(LPN), Tonia (LPN), Rosa (CNA), and Suzanne. These individuals were truly gifts from above! They were professional, yet genuinely kind, caring, compassionate, and attentive. They had a calming demeanor and were just a delight to chat with. No matter how busy these employees got/how many tasks they had to accomplish before they left for the day, they remained calm, pleasant, and on their game. Individuals like this are far and few. They are assets to that facility and should be celebrated by the administration.
They will not be forgotten by us.

I served on the board of this facility under previous ownership, and I had concerns about its transition. However, when my 94-year-old mother needed long-term care in January, we chose this facility and found it exceeded our expectations.

The administrative leadership, including Dimitri, Patti, and Cassie, was supportive and genuine. The care my mother received was exceptional, especially from James and the staff on unit 300.

While staffing and resources can be challenging, the facility is well-maintained and provides a clean and welcoming environment.

I highly recommend this facility for long-term care. The dedicated staff, including many who have been there for years, consistently go above and beyond for residents and families.

Friendly atmosphere from security at the door to the nurses on the floor. Mom feels safe and comfortable also enjoys the food.

My mother was a long term care patient there until February of 2022 due to Alzheimers. She was there for several years and the staff was always helpful and willing to take care of her needs. I particularly liked how they made sure she would attend social activities and parties, along with her medical needs.

I cannot speak highly enough of the caring staff at this long term nursing care facility. Both my brother and my aunt spent their final years as residents, my brother who was mentally disabled to age 85 and my aunt to age 102. The nursing staff, food services, and, most especially, the activities staff ensured they remained engaged and as active as possible even during COVID. The activities staff maintained contact with them even when they were no longer able to participate in the activities.

I will be forever grateful.

Our mother lives in this facility and considers it her home, and the staff like family. She has been there a year and has a good quality of life, enjoys the activities, the other residents, and the staff that run them. Everyone there is so sweet to her and to us. The medical staff has taken great care of her and contacts us often with updates on her medical issues. During this pandemic, the activities staff and social workers at Jefferson have done what they can to help us communicate with our mother via phone and video calls. My sister and I can feel secure in the knowledge that our mother is being cared for and is in the best place for her.

One of the hardest decisions a child can make for their parents is having to choose a long term care facility. My sister and I had to make the decision for our father and we chose this center. The nursing staff has been compassionate and caring. My father is very pleased with the care he is receiving and is very thankful to everyone!

My dad was at this center for 3 weeks. The unit he was in was very clean and updated. The staff was very helpful and attentive to my dad. The social worker, Desiree, is so great to work with. She would respond to my texts within 5 mins (even on nights/weekends). I pray my dad doesn’t need another stay at rehab but, if he does, I would definitely send him back here!

My mother came to the this facility 2 years ago. She first went in through the rehab department. My mom has MS and it has over the years taken away more and more of her muscle function. She was in the rehab for about 30 days. The rehab workers were great. They were very kind to my mom and did everything the could to help her gain back some muscle strength. I was there every single day watching and helping my mom. I was very happy with the care she received through rehab and with the her therapist and the Dr. that was her main therapist. I wont mention names. But he is amazing and showed my mom a ton of compassion and kindness. Her Social Work on rehab at the beginning was amazing and went to great extremes to help my mom. I was very impressed and happy with her care. She did end up transferring to the Long Term Care to live permanently. This was terrifying for her and for me, to leave her in the care of complete strangers. We were assigned a new Social Worker and she also is amazing and does all she can to make moms life a comfortable and content one. I have had great experiences with management when I have needed some help with things getting worked out. As one other person stated in her review no place is perfect and there will be some things that will need to be addressed but I have never had a situation that has not been able to be resolved. My mom has had very good care at Jefferson. I have to give a shout out to the Activities Department. They are awesome! Those ladies do an awesome job at trying to get my mom involved in social activities. Mom typically does not participate but even so they come to her and talk to her and do her nails and bring her treats. They take the time to do that and that means a lot to mom and me. My moms main nurse and the unit secretary have been a great part of my moms experience there. They have been so helpful to us! I have done my research and have spoken to many friends with loved ones in other facilities and this is the best choice. I have been very involved in my moms care from day one. I have learned to be moms advocate and taking the time to make sure staff know my mom’s unique needs and her personality. Being involved and getting to know everyone has helped moms experience be a good one. It helps the staff as well. One thing I have learned is to not assume people understand your loved ones needs, talk to everyone and share the details. It helps a lot. The biggest concern for me was my moms daily care with the CNA’S. For the most part things go very well. If there is a situation with anything regarding moms daily care I address it and it gets taken care of and we move on. Good communication and understanding have been the key to moms success at this center.
