Program

The mission of our cardiac program is to provide our patients with quality, patient-centered care utilizing the most up-to-date scientific guidelines and standards. We aim to provide comprehensive care to those heart failure patients who are unable to return home immediately after hospitalization. We strive to send residents back home and into the community at a level that enhances their quality of life.

Our program features:

• On-site Cardiologist
• Cardiac Rehabilitation
• Patient Education on Disease Management
• Utilization of Heart Failure Care Pathway
• Assistance from a Multi-disciplinary Team to aid in a safe discharge home
• Clinical Care Team, Rehabilitation Staff, Dietitians, Social Workers, & Visiting Nurses if necessary
